Usage Notes

When to use: Use this pattern when you need to verify a user's legal identity by matching their name, date of birth, and partial SSN against records. Common in benefits applications, tax services, and government account recovery flows.
Accessibility considerations: The form uses <fieldset> and <legend> to group related fields. The date of birth fields are grouped together under a single legend. The SSN field includes an aria-describedby reference to the privacy notice paragraph. An error summary with linked field references is displayed at the top on validation failure. All error messages use ARIA live regions via nys-alert.

Customization points: Add middle name or suffix fields as needed. Adjust validation rules for the date of birth (e.g., minimum age requirement). The SSN field could be expanded to collect the full 9 digits if needed. The privacy notice copy should be reviewed by your agency's legal team.
