Missing Documents
The following items are required to proceed with your application. Check each item as you complete it.
If you are unable to submit the required documents by the deadline, your application may be placed on hold or denied. However, you can request a one-time extension of up to 14 days by clicking the "Request Extension" button above. If your application is denied due to missing documents, you may reapply at any time. For urgent situations, please contact our support line at (518) 555-0100 during business hours.
We accept PDF, JPG, JPEG, and PNG files. Each file must be under 10 MB. If you are scanning documents, please ensure the text is legible and the entire document is visible. Photos taken with a smartphone camera are also acceptable as long as the document is clearly readable.
Yes, you may mail physical copies of your documents. However, please note that mailed documents may take 5-7 business days to process, which could cause you to miss the deadline. If you choose to mail documents, send them to: NYS Benefits Processing Center, PO Box 12345, Albany, NY 12201. Include your case number on all documents.
